Function. Apolipoprotein B is the primary apolipoprotein of chylomicrons, VLDL, Lp(a), IDL, and LDL particles (LDL - known commonly by the misnomer "bad cholesterol" when in reference to both heart disease and vascular disease in general), which is responsible for carrying fat molecules (), including cholesterol, around the body to all cells within all tissues. ApoBs are proteins found in lipoprotein particles that are artery-clogging. The apoB-containing lipoprotein particles that are the most damaging to our arteries include not only LDL cholesterol but also remnants of chylomicrons and VLDL (very low density lipoproteins). All three – LDL, VLDL, and chylomicrons – promote atherosclerosis.

However, whether the risk associated with apoB lipoproteins varies with age has not been well examined. Those who had high apolipoprotein B levels, along high total cholesterol and lower HDL levels, saw a 160% increase in coronary artery disease.5 These studies and others conclude that elevated levels of apolipoprotein B is a more reliable marker for cardiovascular disease than LDL, total cholesterol, and HDL levels.
